Processing service provisions begins following the completion of 2 original Order Datasheets (one is for the third-party applicant of the service and the other is for the UMH).

Order Forms must contain the following information:

•    Applicant’s data;

•    Responsible investigator’s (RI) data;

•    A concise, clear and complete description of the activity to be done;

•    Approximate cost;

•    Approximate period of execution.

They must be signed in the designated areas (with original signatures) by the contractor, the responsible investigator and the department/institute director where the responsible investigator is assigned.

Completed and signed Order Datasheets are then sent to the Knowledge Transfer Service for processing.
